Client Intake Form
Please make sure you complete your client intake form prior to your appointment. This allows me to review your information ahead of time and ensure I can provide you with the best and safest massage experience possible.

24-Hour Cancellation Policy
A minimum of 24 hours’ notice is required to cancel or reschedule your appointment.

Cancellations or rescheduling requests made with less than 24 hours’ notice, as well as no-shows, will be subject to a 100% cancellation fee. The full cost of the scheduled massage will be charged to the credit card on file.
