My name is Samuel Miller, and I'm a Licensed Massage Therapist from Florida with several years of Massage Therapy experience in New York. Massage Therapy is my passion and it's why I do it full-time to promote healing and relaxation for my clients.

Please book your appointment under the name of the person receiving Massage Therapy. If you are booking a session for someone else (gift, surprise, etc.), please book under their name of the person receiving the service. If this service is a surprise and/or gift, please text me (850-313-1869) before booking so that we can work together to make the necessary arrangements

A completed, signed and approved Intake Form is required before we can begin treatment. Immediately after booking a session with me, you will receive a link via email to complete an Intake Form. Intake Forms must be completed by the person receiving Massage Therapy. If the person receiving Massage Therapy is under the age of 18, their parent/guardian must be present with them and supervise them at all times while in the clinic, and additional waiver will need to be signed by both the parent/guardian and the client. Intake Forms and SOAP notes for each appointment are considered private healthcare information and are kept on MassageBook's secure server for at least 6 years as required by law.

If you are late for your appointment or have not completed an Intake Form prior to arrival, we won't start on time. If you need to fill your form out in person or need a paper copy, please let me know at least 1 day in advance. If you know you will be late for your appointment, please text me as soon as you know you will be late for your appointment. If I have not heard from you and you are 5 minutes late for your appointment, I will make every attempt to reach you. If you will be more than 15 minutes late for your appointment, we will need to cancel or reschedule and a late fee may apply. If I haven't been able to reach you in 15 minutes, a No-Show fee will apply.

I require a card-on-file for all new clients. This card will NOT be charged UNLESS you have an unapproved Late Cancellation/Reschedule, Late Arrival (>15min) for your appointment, No-Show for your appointment, Walkout before checkout or otherwise behave inappropriately before, during or after your session prior to checkout. 

If you have a discount or coupon/voucher, this will be redeemed post-arrival, pre-treatment, and checkout will be after your treatment. My policies for keeping card-on-file and Intake Form requirement still apply, regardless of whether you have a voucher, and this policy is listed in the voucher's important information tab.

Do not show up for your appointment if you are sick or have a fever within 24 hours before your appointment time, or are under the influence of mind-altering substances or medication that may limit your pain response. Text me as soon as you know you won't make it for your appointment. There is no charge for pre-arranged absences or in cases of sudden illness- only for Late Cancelations and No-Shows. 

Please plan to arrive about 15 minutes early for your appointment and text me when you arrive and I will open the door for you. Please do not knock. 

If you are more than 15 minutes late for your appointment and I have not been able to reach you, I will consider the appointment a No-Call, No-Show, and the cost of the service will be charged to your card on file.

If you need to cancel your appointment due to illness/emergency within 24 hours of the start time, please text me as soon as you know you won't make it. My number is at the bottom of this page.

If you behave inappropriately before, during, or after the massage, the session will end immediately, you will be asked to leave, and the card on-file will be charged for the full amount of the service. A refund will not be processed, and you will be blocked as a client from making future appointments.
