I invite you to make your first appointment. All appointments must be prepaid.
I know that one massage may help you to relax, but a long term plan will foster healing. There are many perks to being a Loyal Member at Ocean Blue Medical Massage and SPA, LLC. I have a Special Discount for Seniors 60 and up, Students, and our Loyal once a month clients.
You will be sent an email intake form to fill out before your first appointment. If you need to fill it out at the office, kindly arrive 15 minutes before your appointment.
Please be sure to arrive 10 minutes early to ensure all paperwork is taken care of and your massage is not delayed!
Cancellation Policy
If you need to reschedule for whatever reason, and especially if you are not feeling well, we understand and request for you to please contact us 24 hours in advance. There is a $50 Cancellation Fee, if cancellation is less than 24 hours. If you have prepaid then you will receive a store credit for the remainder of the amount.
Tardiness Policy
In order to accommodate all of our clients and give them the same courtesy and treatment, if you are 15 minutes late, your treatment will be shortened. If you would like the full treatment time, then you will have to reschedule your appointment and a cancellation fee of $25 will be charged.
Sickness Policy
Massage/bodywork is not appropriate care for infectious or contagious illness. Please cancel your
appointment as soon as you are aware of an infectious or contagious condition. If it is within the 24-hour
notice period, the cancellation fee will be waived. If this office is providing billing services, please be advised of our billing policies.
Cancellation and missed appointments
We do not bill insurance companies for missed appointments or late cancellations. You are responsible for paying the missed appointment/late cancellation fees.
Financial Responsibility
Once your insurance is verified, we will bill and accept payment from your insurance company for covered services. In the event that the insurance company denies payment or makes partial payment, you are responsible for the balance. Your signature confirms your financial responsibility for all services regardless of insurance reimbursement, unless otherwise stated by Lawyer or Vehicle Insurance contract
We accept Letter of Protection from your Lawyer, payment of $50 must be prepaid at each scheduled appointment.
Medical Massage is $200 for Worker's Comp, Auto Collision Injury, Personal Injury with SOAP NOTES.
The amount that I charge is for my years of knowledge and education, and my continuing education, which aids in my learning of new and improved ways to help to achieve a decrease in pain and increase range of motion to bring your body back to homeostasis.
Assignment of Benefits
Your signature authorizes and directs payment of medical benefits to the massage/bodywork
practioner for services provided by this office.
Release of Medical Records
Your signature authorizes the release of all of your medical records on file in this office, for the
purpose of processing your claims, to the following: your attorney, the healthcare providers attending to
this condition, and the insurance case managers. Medical records will not be edited unless otherwise stated in an exclusive release of medical records signed through your attorney.
