UPON ARRIVAL FOR APPOINTMENT

Your intake form MUST be filled out prior to appointment (online).
A mask will be required and need to be on before entering the studio. If you do not have a mask, one will be provided. The mask will need to be worn during your appointment.
Door should be open or unlocked for appointment, if not please knock or send a text.
You will have your temperature taken, contactless, and if there is a temperature of 100F or higher, you will need to reschedule.
Shoe covers may be provided if raining.
PLEASE bring minimal items with you.
Please wash hands or use hand sanitizer after entering.
