INTAKE & CONSENT FORM

Once you schedule your session, you will receive an email from MassageBook with a link to complete your intake and consent forms. Please submit these at least 24 hours before your appointment so I have adequate time to review your health history, contact you with any questions and prepare for your session.

REFUNDS & CANCELLATIONS

Your scheduled session reserves dedicated time specifically for you. Because of this, same day cancelations are non-refundable. If you need to cancel or reschedule, please provide at least 24 hours notice. Payments for canceled sessions will remain in your account as a credit to be used toward a future appointment.( Refund requests will be considered on a case-by-case basis, and processing fees will apply.)
