An intake form will be emailed to you. Please fill out these forms prior to your appointment.
Aim to arrive between 5-10 minutes before your scheduled appointment. This time to allow us to have a verbal intake assessment, as well as time to discuss treatment goals.
Late arrival policy: Life happens! Late arrivals will receive a shortened treatment time that will end at the original designated time. These appointments will not be discounted. Arriving more than 15 minutes late with no communication will be considered a no show.
Cancellation policy: Your appointment time is reserved especially for you. As a solo practitioner, late cancellation and no-shows directly impact my ability to serve other clients. Clients may cancel or reschedule their appointments with more than 24 hours notice. Cancellations made with less than 24 hours notice will result in a $50 cancellation fee. This fee helps compensate for reserved time that cannot be rebooked on short notice. Late cancellation fees must be paid before booking future appointments. Professional and respectful environment: Massage by Terry is a professional massage business. Any and all forms of inappropriate behavior will not be tolerated. This includes sexual solicitation, insinuation, gestures or jokes. In these instances, your session will be immediately terminated, you will be charged the full amount of service, and you will be refused any future services.
