I still cannot bill insurance directly, and can only provide receipts with the relevant information.

Hello! My name is Matt and I started as a massage therapist in 2020 at Rejuvenation Salon & Spa, but I have been licensed since late 2019. Since the closing of that location, I've joined others from that same team over at North Light Salon located in Auke Bay all operating as independent businesses at one location.

A 24-hour notice is required to cancel or reschedule an appointment. Failure to cancel appointments at least 24 hours in advance or failure to show up for your appointment will result in a charge of 50% of the scheduled appointment fee which will be processed on your next visit.

If you need to reschedule because you are not feeling well for any reason, please contact me as soon as possible to reschedule or cancel. There will be no fee charged for this due to illness.

Note on Gift Certificates: They can only be used for services that I provide.

Starting January 1, 2025, those who are requesting superbills will have their own service code "Medically-referred massage" instead of how I had been generating the receipts previously.
